The Senior Company is looking for Licensed or Certified Social Workers who have experience working with seniors for our Mint Care Management Program. With this as a Per Diem position, we can work with your availability so you can work as much or as little as you would like! This is a field-based role serving Hudson County, NJ, involving regular travel to clients’ homes, medical appointments, and community care settings.

Requirements:
Must be a Licensed or Certified Social Worker in New Jersey
Must be able to drive
Experience working with older adults and understanding of aging-related issues

Responsibilities Include:
  • Conduct in-home assessments and develop individualized care plans
  • Coordinate medical and non-medical services
  • Accompany clients to doctor appointments and help manage medical follow-ups
  • Perform regular in-person or phone check-ins to monitor client well-being
  • Communicate with family members and healthcare providers as needed
  • Run light errands (e.g., groceries, prescriptions) to support clients’ daily living needs
  • Advocate for clients in healthcare, housing, and legal settings
  • Provide education and support to families navigating elder care issues
  • Maintain clear, accurate documentation and case
  • And more as long as what is being requested falls within the scope of the LSW license and company policy.
Pay Rate:
$50-$75 per hour
